Staying steady — a quick group try-out

Thanks for helping us test this. In a moment you'll each do a short steadiness check on your own phone — today we're mostly seeing how quick and easy it is to get through.

What is the "Stay Independent" check?

It's 12 short yes-or-no questions from the CDC about everyday steadiness — things like whether you've had a fall in the past year, or feel a little unsteady now and then. There are no right or wrong answers, and it usually takes just a couple of minutes.

Trusted, and quick

This isn't something we made up — it's the CDC's own Stay Independent check, part of their national STEADI program. Doctors, nurses, and pharmacists use it as a quick first step to talk with older adults about staying steady on their feet. It's a friendly starting point for a conversation — never a diagnosis.

How it works

Tap Yes or No for each question. When you finish, you'll see how long it took you. Nothing you answer is saved — we're only timing it.

When everyone's ready, tap Begin to start.

A higher number just means more everyday steadiness factors are present right now — it's a snapshot of today, not a verdict.
How we'll use it

In the Rx360 app, your check-in becomes a starting point — it helps personalize how closely your Balance Meter pays attention to changes in your steadiness over time. A higher starting score just means we watch a little more closely. Today we're only testing how the questions feel — nothing you answered is saved.

One quick favor — how did that feel?
Thank you — that helps.

Source: the CDC's STEADI "Stay Independent" 12-item screen — questions 1 and 2 (a fall in the past year; using or being advised to use a cane or walker) count 2 points, the other ten count 1 point each; a total of 4 or more is the CDC's prompt to talk with a provider (cdc.gov/steadi). This is an educational steadiness check, not a diagnosis. No names or answers are saved — only the completion time, on this device, for today's session.
